About Tricia
I help older adults and their families navigate one of life's most challenging transitions with greater dignity, preparation, clarity and choice.
As Founder of Senior Transition Solutions LLC, I provide personalized senior transition navigation for individuals and families facing planned or urgent moves to higher levels of care. My role is to help transform an overwhelming collection of decisions into a thoughtful, manageable roadmap—while keeping the senior's wishes, priorities and dignity at the center of the process.
My perspective is both professional and personal. My background in medicine has allowed me to see the challenges surrounding senior transitions from the healthcare side. I have also experienced these decisions firsthand while helping my mother over the years and navigating similar challenges with my mother's older brother.
I assist families with transition planning, family meetings, senior-community navigation and tours, comparison of options, coordination of services and resources, downsizing decisions, and the many practical details that can stand between recognizing that a change is needed and successfully making that change.
I believe seniors should remain active participants in decisions about their lives for as long as possible. When they cannot speak for themselves, the people who know and love them best should give them back their meaningful voices.
My real estate experience provides an additional resource when a transition eventually involves the family home—but selling real estate is not the purpose of Senior Transition Solutions. The senior and the transition come first.
I also work to educate families and professional partners about the value of planning before a hospitalization, fall, caregiver crisis or rapidly changing need dictates the timeline.
A transition to senior living is not the end of independence. It can be the beginning of a new chapter—one approached with dignity, preparation and choice.
